Springdale Jr-Sr High School sports physicals for the 2025-26 school year will be held on May 3rd, 2025, in the Alle Kiski Medical Associates Office in Cheswick. Any student, entering grades 7-12, who is interested in playing a school sport during the 2025-2026 school year, is encouraged to schedule a physical.

Dr. David Shin, the school physician, will be available for the sports physical exams.
Please follow the TWO steps required to prepare your child for a school sports physical:

DIRECTIONS to time slot in google calendar: Click on the google calendar link (above). When the google calendar appears, find the May 3rd calendar date. At this point, there will be appointment slots. Please pick an appointment time and type your child’s name in the provided field.
2. Download and complete the nine page CIPPE form. There are required signatures for parents and students. The student needs this form when attending the sports physical appointment. (CIPPE form: https://www.piaa.org/resources/forms/default.aspx )
